The DOL recently issued new FMLA forms on its website. While utilization of the new forms is optional, the DOL explained that the primary purpose of the changes was to make the forms easier to understand for employers, leave administrators, healthcare providers, and employees seeking leave. In particular, the DOL changed the following FMLA forms:
- Rights and Responsibilities Notice, WH-381
- Designation Notice, WH-382
- Employee’s Serious Health Condition, WH-380-E
- Family Member’s Serious Health Condition, WH380-F
- Qualifying Exigency, WH-384
- Military Caregiver Leave of a Current Servicemember, WH-385
- Military Caregiver Leave of a Veteran, WH-385-V
